Parent Survey

Your feedback is requested! Please share your thoughts, feelings and attitudes about your child's school by completing a survey. Please participate! See below for a brief explanation and click on the link to open the survey.

In order to provide programs and services that meet the needs of your family, the Uniondale UFSD would like you to take just a few minutes and complete this online survey.  Your responses will be used to provide educational and fun after school and summer activities to ensure that your children grow up safe and healthy and prepared for success in school and in life.   In addition, parent responses will be used to design activities to help you better support the education of your children including fun activities that you can do together with your children, learning English, and/or learning a different skill to help you advance in your job, find a new job, or just have fun.
